Who needs a DD form 1857?
This form is used to request storage of personal property for more than 90 days. A soldier or an officer may request this government-paid storage by submitting a form 1857. It is issued by the Department of Defense.
What is DD form 1857 for?
The data obtained on this form is used by the installation transportation officer to determine whether the member is entitled to additional temporary storage after the initial 90 days.
Is it accompanied by other forms?
When the applicant provides a reason for this request, such as serious illness or unavailable civilian housing, evidence will be required. This means that medical records as well as documents regarding the applicant’s place of residence must be attached to this form.
When is DD form 1857 due?
Chapter 406 of the Defense Transportation Regulation states that requests for additional storage must be presented well enough in advance of entitlement expirations to allow normal disbursal of property at the expiration of the entitlement.
How do I fill out a DD form 1857?
Section I of this form is just a reference paragraph. Applicants must read it before filling out this form.
Section II is a Member’s Statement of Facts. Applicants check in the box the reason for a second 90-day period of commercial storage. After that they have to furnish their personal information and contacts.
Section III is dedicated to Approval. The name and address of the storage facility are to be filed in the top box. The transportation officer must give his contacts in the bottom of this section.
